The Director of Patient Services/UAS Nurse is responsible for planning, organizing, directing, and coordinating healthcare and professional services of the Licensed Home Care Service Agency and the Assisted Living Program. This position oversees the day to day clinical operation of home care in conformance with the organization's mission, philosophy, and goals. The Director of Patient Services ensures the services he/she is responsible for is delivered in an efficient and cost-effective manner maximizing customer satisfaction. Ensures that all aspects of home care/programs operate within the standards of all applicable laws, rules, and regulations.


  • Ensures the appropriate staff to patient ratios are in place to facilitate quality care.
  • Ensures that 24-hour coverage/service availability is maintained on an ongoing basis.
  • Overall responsibility for the coordination and integration of LHCSA and Assisted Living Program.
  • Demonstrates strong clinical skills and able to oversee direct care staff in their performance based on accepted standards of practice.
  • Participates in the development and implementation an effective staff training and development program for program personnel. Identifies areas in need of growth, provides opportunities for growth and conducts competency/proficiency reviews.
  • Responsible for the interviewing, hiring and supervision of all home care staff.
  • Participates in program development as per the organizations strategic plan, i.e. home care being rolled out in other Bristal communities in the future.
  • Monitors patient care activities to ensure competent and compassionate care is provided to patients.
  • Assists with maintaining compliance with all standards, rules, regulations, and DOH requirements.
  • Provides employees, residents and families with feedback on a regular basis.


  • Currently licensed as a NYS Registered Professional Nurse.
  • Graduate of an approved school of nursing and holds an Associates, Baccalaureate, or Master’s Degree.
  • Two (2) to four (4) years’ experience in a patient care setting with an Assisted Living Program. Medicaid ALP experience is a must!
  • Two (2) years of experience in a supervisory or administrative position in a Licensed Home Care organization
