At the present time, we are looking for a qualified Case Manager/Resident Relations Director to join our beautiful brand new West Babylon, NY start up community. The Case Manager/Resident Relations Director will act as a liaison between Residents, their families, administration staff and community based service providers.

Additional responsibilities include:

  • Acting as support to Residents, providing orientation, social/emotional support and arranging for needed services
  • Oversee the Wellness Department, including a dedicated dementia unit
  • Assess prospective Residents physical, mental and emotional needs and current status
  • Oversee the admissions and discharge process; maintaining accurate case notes and assists Residents in further placement upon discharge
  • Must develop and keep accurate client records, logging Resident interactions and/or impressions and maintaining requisite case notes on Residents
  • Responsible for coordinating ISP process, Incident Reporting, Falls Management Program as well as coordinating outside services such as Rehab, Homecare, Psychological Services.


  • A current NYS RN or LPN license
  • Experience working in an assisted living, skilled nursing, nursing home or long term care facility/environment
  • A successful background in a case work, social work or health advocacy role
  • Minimum of four(4) years experience with an adult population in a social work or discharge planning position preferred
  • Ability to understand the needs of a senior and dementia based population
  • Top notch communication skills, both verbal and written
  • Strong attention to detail, exceptional follow through and time management skills
  • Highly organized, ability to multi-task and thrive in and extremely fast paced, ever changing environment
  • Exceptional interpersonal and relationship building skills
  • Ability to converse will all with a diverse audience in a clear and concise manner
  • Demonstrated leadership skills
  • Schedule to include one weekend day per week for Sunday-Thursday or Tuesday-Saturday schedule. The hours will be 10:30am-7pm or 11am-7:30pm
